Registering as a New Patient

Welcome to Center For Primary Care! We are pleased that you have chosen CPC and we look forward to being your healthcare provider.

Before your first appointment with a CPC physician, you will be asked to complete several forms as part of the new patient registration process. You may do so either at CPC on the day of your appointment or at home beforehand.

If you want to register prior to your appointment, you can print the forms below and complete them by hand, or, if you prefer, you can complete them on your computer and then print and sign them. When you’re finished, please mail them to your new physician’s office or bring them with you to your first appointment. When you check in, you will receive a copy of our office policies, which you will also be asked to review and sign.

Even though you will be asked to provide detailed information about your health history on one of the registration forms you complete at the office, your doctor will probably inquire further about some of them during your first appointment. Please be patient with this process. It is important that we are well informed about your health status in order to provide you the best possible medical care.


If you have any questions about the registration process or your first appointment, please contact the office where you will be seen and one of our staff will be happy to help you. Please remember to bring your current insurance card and any medications that you are currently using with you to your first appointment.
